   I, LEIFS CUSTOMER XXX declare:

1.    I am the owner of a 2000 Subaru Outback, VIN 4S3BH6652Y7649289.



2.    In early 2001, my vehicle was involved in an accident. I made a claim with YYY Insurance Company for the damage.



3.    Shortly thereafter, a woman contacted me and recommended that I take my vehicle to ZZZ Collision Center for repair. She told me that ZZZ Collision Center is a YYY Insurance Company shop and that they do good work. Based on this and the impression I received from YYY Insurance Company about the work of YYY Insurance Company’s shops being guaranteed, I chose to have my vehicle repaired by ZZZ Collision Center.



4.   In October 2004, my vehicle was stolen and damaged. I made a claim with my insurer, YYY Insurance Company, for the damage. A YYY Insurance Company representative recommended that my vehicle be repaired by ZZZ Collision Center because it was a YYY Insurance Company shop. Based on my conversations with YYY Insurance Company, it was my impression that the work of YYY Insurance Company’s shops was guaranteed. However, I decided to have my vehicle repaired by Leif’s Auto Collision Center based on a positive recommendation about Leif’s from a friend.



5.   I have since learned that the original repairs performed by ZZZ Collision Center in 2001 were performed so poorly and improperly that YYY Insurance Company has now decided to declare my vehicle a total loss because of the high cost to correct the repair work by ZZZ Collision Center.

      I hereby declare that the above statement is true to the best of my knowledge and belief, and I understand it is made for use as evidence in court and is subject to penalty of perjury.
